Aviation Safety Reporting System

The Aviation Safety Reporting System collects reports from pilots and other aviation professionals about safety-related issues and events. Pilots often refer to these reports as “NASA Forms” because the system is managed by NASA on behalf of the FAA.

Aviation professionals reporting unintentional violations via the ASRS are less likely to have their FAA certificate suspended or revoked following an incident. For more information or to file an ASR, go to: https://asrs.arc.nasa.gov/
