For all FAA Medical Certifications, the agency now uses an online application via: https://medxpress.faa.gov/medxpress/
Below is a copy of the paper application previously used by the FAA. Pilots and future pilots should review the paper application before beginning an online application. This will provide an opportunity to collect information required for the application and determine the correct entries to be made. Additionally, the instructions attached to the paper form provide more substantial guidance to applicants than the online application.
The National Driver Register is a federally maintained database that tracks significant driving violations, including DUIs, Reckless Driving, License Suspensions, and Revocations. (Minor infractions, such as speeding tickets, are not usually tracked by the NDR.)
The purpose of the records system is to prevent questionable drivers from avoiding penalties by jumping between states. The NDR is maintained using information provided by the states to the national database. State vehicle agencies will review the NDR before issuing a driver's license.
Additionally, the Federal Aviation Administration accesses the database when issuing a medical certification for a pilot.
Members of the public may request a copy of their records via this link https://www.nhtsa.gov/content/ndr or by sending a written (and notarized) request. Below is a Q/A sheet on the NDR and two templates that may be used to request (in writing) a copy of your records.
Individuals may request a copy of their state criminal records by contacting the respective state agency.
